About 5 years ago I was approached  by the Royal College to do a paper on the endodontic implant interface. At the time the conjecture about the longevity, survival and success of implants was starting to being questioned both within the academic field and the service users. This paper attempts to review the evidence for the maintenance of teeth and to explore those situations where the decision to maintain or extract and replace arise.
